security: remove .env from git tracking and add to .gitignore
...
BREAKING CHANGE: .env file is no longer tracked in git for security.
Each environment must maintain its own .env file with secrets.
Use .env.example as a template.

- NextAuth v5 uses AUTH_URL instead of NEXTAUTH_URL
- Keep both for backward compatibility

Add AUTH_TRUST_HOST environment variable for Docker

- Set AUTH_TRUST_HOST=true in .env and docker-compose.yml
- Ensures NextAuth trusts all hosts in Docker environment
